During the U.S. Open in September 2015, Canadian professional tennis player Eugenie Bouchard was in the locker room getting ready to take an ice bath when she slipped on a mysterious substance and cracked her head. Bouchard suffered a concussion and was forced to withdraw from the Open, despite having a shot at winning either the singles or the doubles bracket.

Bouchard later found out that the mysterious substance was a chemical cleaning spray that had been over applied. Not only that, but the room was dimly lite, and Bouchard had no way of seeing the spray on the floor before it was too late. Bouchard claimed the United States Tennis Association (USTA) was negligent in maintaining the locker room.
